Proactive versus Rank-Down Topical Corticosteroid Therapy for Maintenance of Remission in Pediatric Atopic Dermatitis: A Randomized, Open-Label, Active-Controlled, Parallel-Group Study (Anticipate Study)

Topical corticosteroids are used as first-line treatment for atopic dermatitis (AD). Regarding the maintenance of remission achieved by topical corticosteroids, no previous studies have compared proactive therapy with rank-down therapy. We compared their efficacy and safety in Japanese children with moderate to severe AD.

    While I strongly agree with the logic of proactive use of stronger topical steroids as needed in order to ultimately maximize symptom relief and possibly minimize total use of steroids, it might be a tough sell with some parents/patients. A study such as this may help us overcome "steroid phobia" that some patients/parents have.

    Ultimately, the goal should be to reduce itching and clear skin as quickly as possible in order to minimize the overall steroid potency and length of treatment in all AD patients, but especially children, and thereby minimize steroid-induced side effects such as atrophy and hypopigmentation. Patient/parent compliance is often a major barrier to achieving a rapid improvement as well.
